Each row is on the list of most common side effects (in 5% or more of people in the trials) on at least one of these labels: Ozempic, Wegovy, Rybelsus, Mounjaro, Zepbound. The first five rows are on all five lists.

It shows your prescriber which side effects came, on which days and for how long, which is hard to remember at an appointment. Bring the sheet with the dates filled in.

How do I save it as a PDF?
